Notice of Privacy Practices

1. Commitment to Privacy
Your privacy is important. Bloom Counseling and Consulting LLC is required by law to protect your Protected Health Information (PHI) and provide you with this notice explaining how your information may be used and your rights. Bloom Counseling and Consulting LLC is required to follow the terms of this notice currently in effect and may update it as permitted by law. Updated versions will be available upon request.
This notice also explains your rights under HIPAA and federal confidentiality laws protecting Substance Use Disorder (SUD) records, when applicable.
2. Uses and Disclosures of PHI
Your PHI may be used or shared without your written authorization for:
• Treatment, including coordination of care
• Payment, including insurance billing and verification
• Health care operations, including administrative and quality improvement purposes
• When required by law, including abuse reporting or court orders
• Emergencies involving serious risk to your health or safety or that of others
3. Uses and Disclosures Requiring Authorization
Uses and disclosures not listed above require your written authorization. You may revoke your authorization
at any time in writing.
Your information will not be used for marketing or sold.
4. Special Protections for Substance Use Disorder Records (42 CFR Part 2)
Federal law provides additional privacy protections for Substance Use Disorder (SUD) records under 42 CFR
Part 2.
If your record includes SUD information, it generally cannot be shared without your written consent unless
permitted or required by law.
You have the right to:
• Consent to or refuse disclosure
• Revoke consent in writing
• Request an accounting of disclosures
• File a complaint if you believe your privacy rights were violated
5. Your Rights
You have the right to:
• Request restrictions on certain uses and disclosures
• Request confidential communication
• Access and receive a copy of your health information
• Request corrections to your health information
• Receive a copy of this notice
6. Complaints
If you believe your privacy rights have been violated, you may file a complaint with Bloom Counseling
and Consulting LLC or with the U.S. Department of Health and Human Services. You will not be
penalized for filing a complaint
